To upload a file, simply click browse and search for the file on your hard drive. Upload is restricted to a single file. If you wish to send more than one file, please send them as a zip file, ensuring the size of the Zip file is 5Mb or less.

If you're using Windows XP, it already has the ability to create zip files. To create a zip file, select the files you want to add. Then, right-click the files, select "Send To" and "Compressed (zipped) Folder". This will create a .zip file in the same directory which can then be attached to this form.
